You may wish to review this forms and familiarize yourself with Alaska's laws on advance directives. In Alaska, the decision is yours to make and your personal wishes will be followed. Last year, Rep. Bruce Weyhrauch of Juneau sponsored legislation revising Alaska's Living Will statute that gives Alaskans the right to make the decision; however, if you wish to make these decisions, they should be in writing. The attached form walks you through the process with instructions and the forms that are provided for in Alaska Statutes.
